Research Data File Instructions and Data Use Agreement

The Research Data File is generated for public use. Access to the research level version of the inpatient or out-patient data requires the submission of a Data Request Application to the HSCRC. All requests for Research Level Data files are reviewed by the Health Services Cost Review Commission Review Board. The Review Board makes recommendations to grant access to the data at its monthly meeting. The review process may take up to 30 days from submission of the complete letter of request and supporting materials to the Commission for consideration. At the Commission's sole discretion, the investigator may be invited, at the investigator's expense, to appear before the Board to discuss their application. The Board makes the final decisions on the release of the Research Level Data sets. The role of the Review Board is to review applications and make recommendations for final consideration.

Research Data Use Agreement forms must be signed by all users of patient-level data files. A data use agreement (See, Data Use Agreement for the Research Level Statewide Inpatient and Outpatient Data Files) specifying compliance with HSCRC data use restrictions, as well as state law and regulations, pertaining to the use of medical information, must be signed each year.

Statistical Request Form and Data Use Agreement

The Statistical Request Form and Statistical Data Use Agreement are required by all users requesting statistics data. Examples include average length of stay, average charges, counts of discharges with a specific diagnosis. If requesting information on a diagnosis or procedure you must specify specific codes in the section "Specific Diagnosis or Procedure Codes. Requests not specifying codes will not be filled. You may download the complete list of ICD-9-CM and DRG codes from www.cdc.gov. The Data Use Agreement is required by all users requesting the statistics data (see, Statistical Use Agreement for a Statistics Request) specifying compliance with HSCRC data use restrictions as well as state law and regulations pertaining to the use of medical information.
